I am thrilled to interview the experienced Marion Whigham, BSN, RN, who specializes in case management. This informative conversation will discuss the critical issue of adhering to medication regimens and prescriptions.
Effective medication management is critical for sustaining one’s health and well-being, especially for people on complex treatment programs. Our interviewee will graciously give helpful hints, practical tactics, and professional advice on staying organized, sticking to medication regimens, and maximizing the advantages of prescription drugs.
A: I have been a Registered Nurse for a little over 13 years. I am currently a case manager for a company that assists patients with chronic health conditions.
My duties include conducting telephonic visits with patients to determine their needs (medically, financially, etc.). I also assist in coordinating patients’ care, including scheduling follow-ups, arranging proper transportation to and from important appointments, and coordinating with other disciplines (Social workers, Physicians, etc.) to ensure patients’ needs are met.
A: I am a big advocate of patients utilizing pill planners. Pill planners are organizers that have compartments labeled with the days of the week to assist patients in making sure medications are taken correctly.
During my years as a home health nurse, pill planners proved to be an effective tool in helping patients manage their medication regimens.
A: Effective strategies for managing medications that I have witnessed beneficial include picking a designated day every week to refill the pill planner.
Before a refill in needed, the patient can easily gauge the medication remaining. Having a designated area to store your medications at home is an effective strategy for keeping medications organized.
A: Many physician offices now utilize electronic health records. This platform allows the patient to have a more hands-on approach to managing medications, including requesting refills or submitting questions pertaining to a particular medication.
A: Patients can involve family members or caregivers in medication management by allowing a designated person to refill the pill planner on a designated day of the week.
Patients may also want to provide a trusted family member and/or caregiver with access to their electronic health record. Especially if they cannot manage medications or other issues.
In addition, if available, a trusted family member and/or caregiver may accompany the patient to physicians’ appointments. This will ensure the patient understands the information discussed.
